1. Doing the wrong things. You might be doing the same things over and over again. We don't want to spend all day on your website, designing graphics, editing music, etc. You need to be out in the community, evaluating your marketing campaign, analyzing your financials. Delegate the smaller tasks to your staff.

2. Doing things the hard way. Optimizing your system so that it is efficient and tweaking the weak spots. Could you automate some of your process? 

3. Not following-through. Hold onto that fabulous idea and do the work to get it done. Follow your gut and your intuition.
